Internship Paperwork

Services available:

• Internship - Students who want to do an internship for credit fill out this form.

What kind of support can I receive regarding these services?

• There are directions on the form on what to fill out.

• The internship application allows the student to apply to get credits for their internship.

• Agreement has to be signed by the student and the supervisor.

• Student enters in hours worked.

• Final approval will be done by faculty signature and an evaluation by supervisor.

• Career center will review information.

• Registrar will apply credit after everything is approved by others.
