Fix Blank Page Instead of Etrieve Form

If you are trying to get to an Etrieve form and you get a blank page like this:


Click on the hamburger menu in the top right corner to expand the menu.

Click on the Sign Out button

Click the Log Back In button to re-authenticate

Click on the Forms Menu, then find the form you are looking for in the list of available forms and select it

This will generally fix the issue.  If it does not please submit a help desk ticket for further assistance.
