Microsoft Forms allows you to create a poll or quiz that can be shared in Microsoft Teams.

Step-by-step Instructions for Building a Microsoft Form in Microsoft Teams

  1. Go to https://forms.office.com/

  2. Click Sign in

  3. Select a form type.
    1. New Form will create a survey where members of the team can submit multiple responses by default.
    2. New Quiz will create a survey where members of the team can submit a single response by default.

  4.  Add a title for your form.

  5. Click the Add new button to add questions to your form.

  6. Select the question type (e.g. choice, text, rating, date)

  7. Update the question text.

  8. Add question options.

  9. When you're done adding questions, select the More form settings (...) button in the upper right hand corner.

  10. Select Settings in the menu.

  11. Uncheck Record name if you'd like to receive anonymous responses.

  12. Go to the App launcher button in the upper left hand corner.

  13. Select Teams in the menu..

  14. Select your Team.

  15. Make sure you're on the Team's General channel.

  16. select Add a tab (+) on the mail tool bar.

  17.  Select Forms in the Add a tab menu.

  18. Select Add an existing form.

  19.  Select your form and leave the drop down box as Collect responses if you want to keep responses private.

This will create a tab on the toolbar and a post in the chat that a form is available:

The results will be reviewable on Forms:
