Create your Poll:
There are some additional settings in Forms. If you would like the responses to be anonymous, un-check the ‘Record name’ box:
Once the poll has been created in Forms, go to Teams to add it to the general channel. Click the ‘+’ on the main toolbar and select Forms from the listed apps:
Click the radio button to select ‘Add an existing form’ and a list of your available forms will populate. Select the required form and leave the drop-down box as ‘Collect responses’. Otherwise the results will be made available to everyone.

- Add a title for your form.
- Click the Add new button to add questions to your form.
- Select the question type (e.g. choice, text, rating, date)
- Update the question text.
- Add question options.
- When you're done adding questions, select the More form settings (...) button in the upper right hand corner.
- Select Settings in the menu.
- Uncheck Record name if you'd like to receive anonymous responses.
- Go to the App launcher button in the upper left hand corner.
- Select Teams in the menu..
- Select your Team.
- Make sure you're on the Team's General channel.
- select Add a tab (+) on the mail tool bar.
- Select Forms in the Add a tab menu.
- Select Add an existing form.
- Select your form and leave the drop down box as Collect responses if you want to keep responses private.
This will create a tab on the toolbar and a post in the chat that a form is available:
